Do not submit this form if you are a conditional resident seeking to remove conditions. Submit Form I-751 to remove conditions on a Green Card obtained through marriage or Form I-829 to remove conditions on a Green Card obtained through financial investment in a U.S. business.

The current edition is dated 12/17/12. USCIS will accept editions dated 08/10/09 and later until April 16, 2013. After April 16, 2013, we will only accept the 12/17/12 edition.

Application Type B: My card was issued but never received.
There is no fee, but you must meet these criteria:
We mailed the card over 30 days ago
It was returned to us as undeliverable
You have not moved from the address you provided us
You must submit a copy of the Form I-797, Notice of Action, for Form I-485, Form I-751, Form I-829, or Form I-90 that resulted in the issuance of your card.
You must also submit a copy of government-issued identification that contains your name, date of birth, photograph and signature.
Application Type D: My card contains incorrect information due to USCIS error.
There is no fee
