Item No.: 08
Site: Potable Water-Trolley Washers
Violation: The reduced pressure assembly for the deck 3 trolley washer had the relief vent capped and the reduced pressure assembly for the deck 4 trolley washer had the relief vent capped and facing up. These assemblies were both heavily corroded. These assemblies were not part of the cross-connection control program and were not tested annually.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Recommendation: Inspect and test backflow prevention devices requiring testing with a test kit after installation and at least annually. Maintain the test results showing the pressure differences on both sides of the valves for each device.

Site: Potable Water-Distillate and Permeate Lines
Violation: The distillate lines from the two evaporators and the permeate lines from the reverse osmosis system were not striped to the production halogen injection point in accordance with ISO 14726 standards (blue/gray/blue). Between the mineralizer and the halogen injection point the lines were striped blue/green/blue, indicating potable water.
Recommendation: Stripe or paint permeate lines directed to the potable water system in accordance with ISO 14726 (blue/gray/blue).

Site: Recreational Water Facilities-Whirlpool Sample Lines
Violation: The sample lines for the electronic data loggers for the whirlpool spas were located after the compensation tank. Staff stated the materials for the sample line relocation were to be delivered on 1 December.
Recommendation: Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank.

Site: Recreational Water Facilities-Swimming Pool Antientrapment
Violation: Each swimming pool had two or more gravity drains greater than three feet apart constructed of a standard design and no alarms were installed. Staff stated new antientrapment drain covers were scheduled to arrive on 17 November.
Recommendation: For RWFs with gravity drainage and multiple drains (2 or more drains greater than 3 feet apart), provide either a standard design (not compliant with ASME A112.19.8) drain cover and an alarm, or an ASME A112.19.8 compliant antientrapment/antientanglement drain cover. Ensure the alarm is audible and sounds in both a continuously manned space and at the RWF. Ensure this alarm is for all draining: accidental, routine, and emergency.

Site: Recreational Water Facilities-Whirlpool Safety Signs
Violation: The safety signs for the whirlpools cautioned against use of individuals with high blood pressure, but not low blood pressure.
Recommendation: In addition to the safety sign requirements in section 6.7.1.1.1, install a sign at each whirlpool spa and spa pool entrance listing precautions and risks associated with the use of these facilities. Include, at a minimum, cautions against use by individuals having underlying medical conditions such as high or low blood pressure.

Site: Recreational Water Facilities-Solarium Pool
Violation: The shepherd's hook was not visible from the full perimeter of the pool. It was located in the towel dispensing room.
Recommendation: Provide a rescue or shepherd's hook and an approved flotation device at a prominent location (visible from the full perimeter of the pool) at each RWF that has a depth of 1 meter (3 feet) or greater. Mount these devices in a manner that allows for easy access during an emergency. Ensure the pole of the rescue or shepherd's hook is long enough to reach the center of the deepest portion of the pool from the side plus 2 feet. Ensure it is a light, strong, nontelescoping material with rounded, nonsharp ends. Ensure the approved flotation device includes an attached rope that is at least 2/3 of the maximum pool width.

Site: Recreational Water Facilities-Whirlpool Drain Covers
Violation: It could not be determined if all of the required information was stamped on the whirlpool drain covers. There were no spare drain covers or documentation onboard and the whirlpools were full of water.
Recommendation: Ensure the following information is stamped on each manufactured antientrapment drain cover: (1) certification standard and year; (2) type of drain use (single or multiple); (3) maximum flow rate (in gallons or liters per minute); (4) type of fitting (suction outlet); (5) life expectancy of cover; (6) mounting orientation (wall, floor, or both); (7) manufacturer's name or trademark; and (8) model designation.

Violation: The ice in the ice dispenser was uncovered and unprotected.
Recommendation: Protect food from contamination by storing the food covered or otherwise protected.

Violation: There were no serving utensils for a tray of grapes and a tray of oranges.
Recommendation: Provide food-dispensing utensils for each container displayed at a consumer self-service unit such as a buffet or salad bar.

Site: Other-Cafe Promenade
Violation: The splash zone above the milk steamers of the San Marco espresso machine was open to wires and pipes, making this area difficult to clean.
Recommendation: Ensure multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams greater than 0.8 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel. Use only low profile, nonslotted, noncorroding, and easy-to-clean fasteners on food-contact surfaces and in splash zones. Prohibit the use of exposed slotted screws, Phillips head screws, or pop rivets in these areas.

Violation: The door hinges of the upright trolley were heavily corroded.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The underside of the San Marco espresso machine was open to wires and pipes, making this area difficult to clean.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The data plates for the five rack conveyor warewash machines indicated speed in racks per hour instead of the minimum transit time.
Recommendation: Ensure a warewashing machine is provided with an easily accessible and readable data plate affixed to or posted adjacent to the machine that indicates the machine's design and operating specifications including the minimum transit time for rack conveyor machines.

Violation: The wires and pipes in the underside of the San Marco espresso machine were heavily soiled with dust and coffee residues.
Recommendation: Ensure nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: Six clean wine glasses were not stored covered or inverted.
Recommendation: Store clean equipment and utensils in a self-draining position that allows air drying, and covered or inverted.

Violation: The handwashing station at the entrance of the in-use galley had no waste receptacle.
Recommendation: Ensure a handwashing facility includes a sink, soap dispenser, single-use towels dispenser, and waste receptacle.

Violation: There was a gap between the bulkhead panels next to the hood-type warewasher.
Recommendation: Close the gap.

Site: Galley-Deck 3 Wine Cellar
Violation: A squeegee was stored under the handwashing station.
Recommendation: Ensure maintenance tools such as mops, brooms, and similar items are stored in a designated locker so they do not contaminate food, food-contact surfaces of utensils, and equipment, linens, and single-service and single-use articles.
