Item No.: 08
Site: Potable Water-Cross-Connection Control Program
Violation: The backflow prevention devices on the multi-flow soda units were not included in the list of backflow prevention devices. For cabin showers and toilets, the list did not indicate the total number of each backflow prevention device.
Recommendation: Ensure the cross-connection control program includes at a minimum: a complete listing of cross-connections and the backflow prevention method or device for each, so there is a match to the plumbing system component and location. Include air gaps on the listing. Ensure a schedule for inspection frequency is set. Maintain a log documenting the inspection and maintenance in written or electronic form, and ensure it is available for review during inspections.

Site: Pantry-Calypso
Violation: There was a continuous leak from the vent on the carbonator backflow prevention device in the soda cabinet.
Recommendation: Maintain backflow prevention devices in good repair.

Site: Recreational Water Facilities-Calypso Pool
Violation: The pH in the Calypso pool was measured between 7.96 and 8.14. There was no one in the pool and it was immediately netted.
Recommendation: Maintain the pH level in all RWFs between 7.0 and 7.8. Immediately close facilities if these halogen and pH ranges are not maintained.

Site: Recreational Water Facilities-Whirlpool Sample Lines
Violation: The sample lines for halogen and pH analyzers for the Calypso, Spa, and Neptune whirlpools were not located directly from the RWF or on the return line from the RWFs. The vessel had corrected some of the sample lines and the remaining lines are expected to be relocated by December 2012.
Recommendation: Ensure the sample line for the analyzer probe (monitoring) is either directly from the RWF or on the return line from each RWF and before the compensation tank.

Site: Recreational Water Facilities-Safety Signs
Violation: The safety sign for the Terrace pool was in a location where is was blocked by pool chairs. The mounted signs at the other pools did not meet the requirements of the 2011 VSP Operations Manual. New signs were onboard and were installed during the inspection.
Recommendation: Provide safety signs for all RWFs, except for baby-only water facilities. Ensure the signs, at a minimum, include the following words: (1) do not use these facilities if you are experiencing diarrhea, vomiting, or fever; (2) no children in diapers or who are not toilet trained; (3) shower before entering the facility; and (4) bather load #. For children's RWF signs, include the exact wording "TAKE CHILDREN ON FREQUENT BATHROOM BREAKS" or "TAKE CHILDREN ON FREQUENT TOILET BREAKS."

Site: Buffet-Trident Grill
Violation: When asked to take the temperature of a hamburger, the cook used a nontip-sensitive thermometer and did not insert the probe completely up to the measuring dimple. In addition, the cook did not sanitize the thermometer probe before using it and had to be advised by the inspector to do so. A tip-sensitive thermometer was available in the area.
Recommendation: Ensure the cooks are trained to use the correct thermometer for thin foods and to sanitize the thermometers before taking temperatures of food items.

Site: Food Service General-Consumer Advisories on Menus
Violation: On the Sabatini's menu, there were no asterisks at the seared tuna, steak, or veal. According to the staff, the steak and veal were cooked to order and could be served undercooked. On the Crown Grill menu, there were no asterisks at the steaks and some of the chops. According to the staff, the steaks and chops were cooked to order and could be served undercooked.
Recommendation: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish that is raw, undercooked, or not otherwise processed to eliminate pathogens is offered in a ready-to-eat form or as a raw ingredient in another ready-to-eat food, ensure the consumer is informed by way of disclosure using menu advisories, placards, or other easily visible written means of the significantly increased risk to certain especially vulnerable consumers eating such foods in raw or undercooked form. Locate the advisory at the outlets where these types of food are served. Ensure that disclosure is made by one of the two following methods: (2) On a menu using an asterisk at the animal-derived foods requiring disclosure and a footnote with a statement indicating that consuming raw or undercooked meats, seafood, shellfish, eggs, milk, or poultry may increase your risk for foodborne illness, especially if you have certain medical conditions.

Site: Buffet-Horizon Court - Starboard
Violation: One stack of plates on the center island was uncovered. This was corrected.
Recommendation: During service, ensure the food contact portion of each self-service food dispensing utensil is covered or located beneath shielding. Ensure that dishware, glassware, and utensils out for service are inverted or covered.

Site: Provisions-Vegetable Walk-in Refrigerator
Violation: Water was dripping from the deckhead onto two boxes of mushrooms.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; and (4) At least 15 centimeters (6 inches) above the deck.

Site: Provisions-Deck 4 Outside of the F&B Office
Violation: Two deck stands with bagged flour were stored under an open deckhead with ventilation ducts, cables and cable trays, and other difficult to clean features. The flour was moved.
Recommendation: Protect food from contamination by storing the food: (1) Covered or otherwise protected; (2) In a clean, dry location; (3) Where it is not exposed to splash, dust, or other contamination; and (4) At least 15 centimeters (6 inches) above the deck.

Site: Galley-Potwash
Violation: There were gaps where tack welds had broken around the stainless steel panel on the underside of the pulper station counter. This was corrected.
Recommendation: Ensure nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Galley-Deck 6 Service Line
Violation: In bain marie technical compartments #4 and #5, the steam pipe insulation and cloth wrap were frayed.
Recommendation: Ensure nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Site: Room Service-
Violation: Several trays of soiled coffee carafes were stored on a trolley labeled 'Laundry Trolley.' According to the staff in this area, the trolley did come from the laundry and was used to store the carafes that would be washed right before service.
Recommendation: Ensure that trolleys from the laundry are not used in food areas for soiled storage.

Site: Galley-Crown Grill
Violation: A wet wiping cloth was on the counter.
Recommendation: Restrict wiping cloths to the following: (1) Cloths used for wiping food spills are used for no other purpose; (2) Cloths used for wiping food spills are dry and used for wiping food spills from tableware and single service articles or wet and cleaned, stored in a chemical sanitizer, and used for wiping spills from food-contact and nonfood-contact surfaces of equipment; (3) Dry or wet cloths used with raw animal foods are kept separate from cloths used for other purposes. Wet cloths used with raw animal foods must be kept in a separate sanitizing solution; (4) Wet wiping cloths used with a freshly made sanitizing solution and dry wiping cloths are free of food debris and visible soil.

Violation: There was no toilet tissue. This was corrected.
Recommendation: Provide a supply of toilet tissue at each toilet at all times.

Violation: Water was dripping from a deckhead seam over two boxes of mushrooms.
Recommendation: Ensure the plumbing system in food areas is maintained in good repair.

Site: Galley-Deck 6 Pastry Distribution
Violation: The condensate drain line for the upright refrigerators was not directed to the scupper and water was pooling on the deck.
Recommendation: Ensure black and gray water is discharged to the vessel's wastewater disposal system and does not pool on the deck.

Site: Pantry-Lighting
Violation: The light intensity beside and behind the ice machines and all inspected pantries was less than 110 lux. This included the ice dispenser in pantry 12110.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at a distance of 75 centimeters (30 inches) above the deck when in use, in walk-in refrigerator units and dry storage areas, and in other areas and rooms during periods of cleaning. Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Site: Food Service General-Lighting
Violation: There were several areas where the light intensity around and behind deck and counter-mounted equipment was less than 110 lux. Areas included: juice and milk dispensers in the Horizon Court area next to the gazebo; behind ice machine in the Horizon Court self-service beverage station #1; around and behind ice machine in the bell box; behind ice machine in the Crown Grill galley; behind combination ovens #1 and #4 in the deck 6 main galley; around and behind ice machine in the deck 6 main galley garde manger; behind toaster in the deck 6 main galley port aft coffee station; behind and on the left side of stack oven in the deck 5 main galley pastry; behind ice machines in the deck 5 main galley starboard forward area; behind equipment on the deck 5 main galley coffee station; behind ice machine in the crew galley; behind equipment on both of the crew mess service lines; behind ice machine in the officers pantry; around and behind ice machine in the International Cafe pantry; and behind ice machine in the Crooner's Bar pantry. Several new lights had been installed since the previous inspection. In addition, the vessel had completed a light survey and created an extensive list of areas that needed additional lighting. Most of the items found during the inspection were on the vessel's list.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) behind and around mounted equipment, including counter-mounted equipment.

Site: Room Service-
Violation: The light intensity was less than 220 lux at the handwash station near the ice machine.
Recommendation: Ensure the light intensity is at least 220 lux (20 foot candles) on food preparation surfaces, and at a distance of 75 centimeters (30 inches) above the deck in food preparation areas, handwashing facilities, warewashing areas, equipment, and utensil storage, pantries, toilet rooms, and consumer self-service areas.

Violation: The light intensity was less than 110 lux at the handwashing station while the bar was in operation.
Recommendation: Ensure the light intensity is at least 110 lux (10 foot candles) at handwashing stations in bars.

Violation: There were two small black flies in the undercounter pulper area for the dishwash.
Recommendation: Effectively control the presence of insects, rodents, and other pests to minimize their presence in the food storage, preparation, and service areas and warewashing and utensil storage areas aboard a vessel.

Site: Integrated Pest Management-Inspection Location
Violation: The IPM active/passive inspection log sheets did not indicate the specific location of inspections. Only the general category such as bars, galleys, etc. was listed unless there was a positive finding.
Recommendation: Ensure that the inspection locations are included so that the time of inspection for each location can be determined.
