Military Students
ALL MILITARY STUDENTS MUST MAKE APPOINTMENTS THROUGH THEIR OPERATIONS SERGEANT. DO NOT MAKE APPOINTMENT THROUGH THIS SITE.
PSDR Units
ALL PSDR UNITS (513TH MI,706th MI,707th MI and 35TH SIG) MUST GO THROUGH THEIR BRIGADE FOR CAC ISSUANCE. DO NOT MAKE APPOINTMENTS THROUGH THIS SITE. HOWEVER, FAMILY MEMBERS CARDS ARE STILL ISSUED HERE.
Cancel/Reschedule Appointment
Customers will need to know their confirmation number to cancel/reschedule appointments. Please keep a copy of your appointment confirmation page. If you are more than 10 min late for your appointment you will have to reschedule.
Lost or Stolen ID Cards
All lost or stolen ID cards must be reported to the Military Police. The Military Police will give you a form to bring to the ID card section.

Documentation
ALL REQUIRED DOCUMENTATION MUST BE ORIGINAL OR CERTIFIED COPIES. ANY FOREIGN DOCUMENTS MUST BE TRANSLATED INTO ENGLISH BY AN OFFICIAL GOVERNMENT TRANSLATOR.
Expiring ID Cards
ID cards will be replaced up to 30 days before expiration date.
Required Documents
Two (2) forms of UNEXPIRED identification, one must have a picture from local, state, or government agency (i.e. drivers license, Passport, Military ID) Birth certificate, Social Security Card.
Power of Attorney
POWER OF ATTORNEY (POA): If you are not accompanied by your sponsor you will need a valid General or Special POA. (Special POA must state ID Card issuance and DEERS Enrollment)
Family Member Cards
For family member ID Cards, the sponsor must be present to sign the DD Form 1172. If the sponsor is not available, the family member must have a DD Form 1172 signed by the sponsor and the ID card facility, or a valid POA.
Full Time College Students (Age 21 - 23)
All College students age 21 - 23, must bring a letter from the registrar's office stating that they are enrolled full time in an accredited college, in pursuit of an Associate Degree or higher, and if possible the estimated date of graduation.
