Billing and Fees
The Data Bank must charge a minimum fee to recover the costs of operating the Data Bank, according to Federal law. The Data Bank does not receive Federal appropriations; i.e., Federal taxes do not support the Data Bank. Payment is safe, secure, and can be made by major credit card or electronic funds transfer (EFT). Any changes to fees are announced by the Secretary of HHS in the Federal Register.
How it Works
Each time you, as a health care organization, submit a query, or when you enroll in Continuous Query - you must pay the required fee. First, however, make sure that your Data Bank Administrator has completed some preliminary steps:
- If you are paying via credit card (VISA, MasterCard, Discover, or American Express) and prefer not to enter your card information each time you query, your Data Bank Administrator may store the authorized credit card account numbers and authorize usage. (See How to Maintain and Store Credit Card Accounts).
- If you are paying via an electronic funds transfer, your Data Bank Administrator must first complete the EFT Authorization Form (available after you sign in to the Data Bank). You must keep your account information up-to-date using the same form.
Fees
Health care organizations are charged fees for queries and for enrollment in the Continuous Query service.
Query Fees
Query fees are calculated on a per-name basis per Data Bank: the National Practitioner Data Bank (NPDB), and/or the Healthcare Integrity and Protection Data Bank (HIPDB). If you are querying on multiple practitioners, the number of names in your query are multiplied by the per-name fee.
Note: Fees are assessed for all queries submitted. This includes both queries that are processed by the Data Bank (regardless of whether there is information on file regarding a practitioner); and queries that are rejected because they are improperly submitted or incomplete.

Continuous Query Fees
The annual enrollment fee for Continuous Query covers the cost of enrolling one practitioner in one Data Bank for one year. (Enrollment expires on the last day of the same month of the following year.)

Keeping Track of Your Account
Your Billing History
To help you keep track of your financial transactions with the Data Bank, you can see your billing history online at any time. (See View Billing History.)
Bill Reference Numbers
To assist you in reconciling your payment transactions, the Data Bank provides a bill reference number for each charge to your organization's EFT or credit card account for a specific transaction. You can find this number on your charge receipt if you are doing IQRS (Integrated Querying and Reporting Service) queries, and in each response file for QRXS (Querying and Reporting XML Service) queries.
Questions or Disputes?
If you have questions about a charge or you believe that you were charged incorrectly, please contact the Data Bank, in writing, as soon as possible. Your questions or disputes must arrive no later than 60 days after submitting the query on which the error or problem appeared. You may submit your request by letter or by printing and completing the Account Discrepancy form (PDF - 128 KB).

Your letter must provide the following information:
- Your name and credit card or EFT account number
- The dollar amount of the suspected error
- A description of the error and explanation of why you believe there is an error
- The Data Bank Control Number (DCN) of the query transaction in question
- Your organization's Data Bank Identification (DBID) number
- Your telephone number
- Your signature
- A copy of your charge receipt
Note:
The Data Bank is committed to protecting your privacy and your Personally Identifiable Information (PII). In accordance with HHS and HRSA policy, the Data Bank will not accept unencrypted PII via email or fax. Either 1) Contact the Data Bank Customer Service Center at 1-800-767-6732 to discuss IQRS Secure Messaging or email encryption options or 2) Send all documents containing PII via US Mail or FedEx.
Outstanding Balances
The Data Bank collects outstanding query fee balances. The Data Bank will request that you complete an Account Balance Transfer Request form (PDF - 128 KB) to authorize settlement of an outstanding balance.
