Reporting PII Breach Notifications

Commands reporting a loss or suspected loss of personally identifiable information (PII) will be contacted by the Department of the Navy Chief Information Officer Privacy Team to determine if individual notifications are required. The decision to notify will be based on the nature of the PII compromised and the resultant level of risk of identity theft. If the command is faced with notifications and cannot locate the current addresses of those impacted, please contact Steve Daughety at steve.daughety1.ctr@navy.mil or Steve Muck at steven.muck@navy.mil for points of contact to assist in locating the individuals in question.
