The 354th Medical Group provides outpatient primary healthcare under the TRICARE program for all eligible beneficiaries living in the Eielson area. In addition, a collocated dental clinic provides general dental care for all active-duty military members.

A fully credentialed staff of family practice physicians, pediatricians, flight surgeons, and general dentists provide care at the Eielson medical clinic. In addition, optometry, physical therapy and mental health support are available.

Pharmacy, laboratory, X-ray and immunizations services are located in the clinic. Bassett Army Community Hospital on Fort Wainwright serves as the Eielson clinic's primary referral source for specialty and inpatient care. Other specialty services are available by referral in the local community and through aero medical evacuation to the 3rd Medical Group at Elmendorf AFB or Madigan Army Medical Center at Fort Lewis, Wash.

The Primary Care Clinic hours are 7:30 a.m. to 4:30 p.m. Mondays through Fridays. The 354th MDG is closed on Federal holidays, Wing goal days and at 1200 on the last Wednesday of the month for mandatory training.
To make an appointment or to leave a request for a telephone consult call 377-1847. For dental appointment you may call either 377-1847 or 377-1846. To contact the Mental health Clinic call 377-3071.
There are no emergency services available at the 354th Medical Group.

As a patient of the 354th MDG please note the following:

Patient Rights and Responsibilities

Access to Emergency Services:
You have the right to emergency care without preauthorization where and when acute symptoms are so severe that a "sensible layperson" would want emergency care to prevent serious harm or death.

Respect and Nondiscrimination:
You have the right to timely, considerate, and respectful care at all times and under all circumstances.
You have the right to an environment of shared respect which is free from discrimination.
You have the right to request a chaperone for any examination process.
You are responsible to have respect for the rights of other patients and MTF staff.

Choice of Providers and Plans:
You have the right to accurate information about the TRICARE program to include covered health benefits and health plan options.
You have the right to a choice of healthcare providers, either within TRICARE Prime network or the Military Treatment Facility (MTF).
You are responsible to become educated about TRICARE coverage, options, and rules, to include required fees.

Participation in Your Care and Care Decisions:
You have the right to receive accurate, accessible and understandable healthcare information in a language you understand and in a culturally-sensitive way.
You have the right to ask questions and receive timely answers to those questions.
You have the right to choose someone to make decisions on your behalf if you cannot do so.
You have the right to know all of your treatment options including the option of no treatment.
You have the right to know the risks, benefits, and consequences to each of these options.
You have the right to refuse care as allowed by law.
You are responsible to tell your healthcare professional everything you know about your illness or condition, past health care, and any and all medications or supplements you are taking.
You are responsible to inform the doctor or nurse about any changes in how you feel.
You are responsible to participate in decisions related to your healthcare and ask questions if you do not understand any part of the proposed treatment.
You are responsible to follow your provider's plan of care.
You are responsible to maximize healthy habits such as exercise, not smoke, eat a healthy diet, and not knowingly spread disease.
You are responsible to accept the consequences if you refuse to follow the plan of care.
You are responsible to keep appointments or call to change or cancel them at least 24 hours in advance.

Confidentiality of Health Information:
You have the right to privacy and protection of your personal health information.
You have the right to communicate with healthcare providers in confidence as allowed by law and regulation.
You have the right to review and obtain a copy of your own medical records.
You have the right to request amendments to your records, within specific legal limits

Appeals and Complaints:
You have the right to a fair and efficient process to appeal medical necessity decisions by your MTF or by TRICARE that includes both internal review and independent external review.
You have the right to discuss complaints with your providers or Patient Advocate.
You have the right to contact a TRICARE Debt Collection Assistance Officer if unable to pay fees.
You are responsible to report wrongdoing and fraud to appropriate authorities.

You have the right to review a provider directory maintained on TRICAREon-line.com website, to assist with understanding your provider's education and training and selecting a Primary Care Manager.
