The Headquarters (HQ) Defense Logistics Agency (DLA) has turned over the distribution responsibilities of the Joint Service Award Certificates to the DLA Document Services.

Effective March 19, 2008, customers will order the joint service award certificates via the Navy Forms Online Web Site. You must establish an account prior to placing orders by registering.

To register, go to https://forms.documentservices.dla.mil/order/ and follow the Registration link in the "Establish An Account" step. Supplying your DoDAAC during registration is important because it not only verifies you as a Government customer, but it also supplies the Naval Forms warehouse with your TAC 2 shipping address, where your orders will be sent. You will be given the opportunity to review this address during the registration process. Once your account is approved, you will receive an email assigning your User ID and Password. If you don't receive this email within a day or two of your application, contact the Naval Forms staff for assistance at 1-877-327-7226.

With your User ID and Password handy, return to https://forms.documentservices.dla.mil/order/ and follow the instructions for searching and ordering forms.
