READ BEFORE YOU PROCEED.

Once you have gathered all the necessary information and have researched the plausibility of your complaint, you should then fill out and submit a Hotline Complaint Form to the 2d Marine Aircraft Wing Inspector.

In order to begin filing your complaint you must:

Download the Hotline Complaint Form by right clicking on this link and
use a printer to produce the document.


• Fill in the blanks using Microsoft Word or print legibly by hand.
• Fill out the form completely. Leave no blanks in the information blocks.
• Then forward your completed form to the Wing Inspector either via e-mail, facsimile, or standard correspondence.
• You will be contacted to schedule a follow-up interview.
• Bring all documentation pertaining to your complaint to the interview.
• The Inspector will then make a determination and address your case.
