To schedule or cancel an appointment with the Primary Care Clinic for enrolled family members or active duty, call 321-494-8241.

Dental Clinic:
To schedule or cancel an appointment at the Dental Clinic for active duty, call 321-494-6366. Try to give 24 hours advance notice for cancellations.

The Dental Clinic is located at 1389 S. Patrick Drive, Patrick Air Force Base, Fla., 32925.

The satellite pharmacy is located beside the Base Exchange behind the Burger King.

Dial 911 for Emergency

The clinic does not provide emergency services

In the event of an emergency, dial 911 or go the nearest emergency room. Patients are asked to contact their Primary Care Manager (PCM) within one duty day to report any care received at an emergency room to ensure proper follow-up.
tabAbout the MTF
The 45th Medical Group provides ambulatory medical service to active duty and their enrolled members.  It also provides Dental medical service to active duty only.

The Medical Group provides the following patient care services: pediatrics, limited gynecological services, immunization, general denistry, physical therapy, optometry, mental health, substance abuse, family advocacy, laboratory and pharmacy. Those services not offered are provided through the TRICARE network.
