Frequently Asked Questions

Question: Who is responsible for numbering all directives for XVIII Airborne Corps, United States Garrison and Fort Bragg installation units/activities.

Answer: The Document and Forms Management Office, Administrative Services
Division, Directorate of Human Resources.

 

Question: What types of publications are numbered?

Answer: Numbered publications include regulations, circulars, memorandums, pamphlets, supplements to higher headquarter's publications, policy or precedent forms and changes to any numbered directives.

 

Question: What directive provides guidance on the preparation of Fort Bragg numbered directives?

Answer: Fort Bragg Memo 25-31, Preparation and Staffing of Fort Bragg Numbered Directives.

 


Question: Who is responsible for assigning office symbols?

Answer: The Document and Forms Management Office.

 

Question: How do I request, change or delete an office symbol?

Answer: Requests may be submitted via email, memorandum or fax, the preferred method is email. Requests must include, POC's name, title, telephone number, email address, correct unit/activity title, and full justification.

 

Question: Can anyone create a form?

Answer: No. Only Activity Forms Coordinators or alternates can create, modify or rescind forms. Coordinators and alternates must be appointed in writing. A copy of appointment orders must be provided to the Document and Forms Management Office.

 

Question: What are the submission requirements for creating, revising or rescinding a form?

Answer: A copy of appointment letter must be on file, DD Form 67 (Unit Forms Coordinator Memorandum), draft, CD copy of draft, and the prescribing directive(s).

 


Question: Where can I find the most current version of the DD Form 67?

Answer: The DD Form 67 with instructions is available on the Army Publishing Directorate website, http://www.dtic.mil/whs/directives/infomgt/forms/eforms/dd0067.pdf.

 


Question: Do I have to submit a DD form 67 for every form that I create, revise or rescind?


Answer: Yes

 

Question: What information must be included on all finalized forms?

Answer: Proponent office, prescribe and directives, title, authority, purpose, privacy act (if the collection of personal data from an individual such as social security number, etc is required).

 


Question: If my form is requesting personal information, what should I do?

Answer: Refer to AR 25-30, para 4-5 for additional guidance.

 

Question: Can I create a form similar to an existing higher echelon form?

Answer: No. The higher echelon form must be used. AR 25-30, para 4-2 states, "A form that duplicates the function of a higher echelon form will not be created".

 

Question: Are there templates available for creating a form.

Answer: No.

 

Question: What is the Forms Content Management Program (FCMP)?

Answer: The FCMP is the new official Army standard for Forms, digital signature, and electronic information processing. The FCMP integrated enterprise solution consists of PureEdge (Workplace) forms and digital signature (ApproveIt) software purchased for the Army enterprise 1.4 million army wide users. The FCMP provides an enterprise solution for providing not only fillable forms and storage of data, but it will also enable a form to be pushed from pc to pc using digital signature and workflow techniques. This program provides the U.S. Army the means to develop one single enterprise solution in automating functional business processes army wide, eliminating duplicative stovepipe systems being developed across the army. In addition, with the ApproveIt digital signature capability in place this will eliminate army resources for being expended on courier, transportation, mailing (FedEx), handling, cost, etc.

 


Question: What is the PureEdge e-form?

Answer: PureEdge e-forms represent a quantum leap over previous forms software and are far more than simply another electronic forms solution. Under the new program, the printed form may be one of many potential outputs of what will comprise the new system. The PureEdge e-form is an intelligent form that encapsulates the form logic and the underlying data into a single file in a non-proprietary Extensible Markup Language (XML) format. Secure electronic signatures, automatic routing of the form to recipients, and support for non-connected users are just some of the benefits of the e-form. Additional features include automatic information population, synchronization of form data with Army databases, and integration with functional business processes.

 


Question: What software do I need to use/read Pure Edge e-forms ?

Answer: You must have Pure Edge software installed on your personal computer to use electronic fillable forms. In order to digitally sign a signature enabled electronic form you will need to install Silanis ApproveIT software. NOTE: At this time, not all forms have been signature enabled.

 

Question: How can I get the PureEdge Viewer and ApproveIt software?

Answer: The Army-tailored PureEdge Viewer v6.5 software, along with the Army-tailored Silanis ApproveIt Desktop v5.7.1 electronic approval software is available through the Army Golden Master IV or via download from the Army Small Computer Program website.
