Patients will need to have a copy of PCS/ETS orders or if being seen at an outside provider will need to supply the name of the facility and Doctor. We can only supply the patient with a print out of the final radiologist reports for their personal records.

How can i get copies of images on a CD?

The radiology file room personnel can burn studies onto a CD by calling the file room at (910) 907-8640. You can also download DD Form 2870 or DD Form 877 Medical Release and fax it to (910) 907-8733 to the file room. Hours of operation are Monday - Friday from 7:15 a.m. - 4 p.m. (Except holidays).
