What should I do if Social Security notifies me my name or Social Security number do not match Social Security's records or my employer's records?

Verify that the name and Social Security number on your Social Security card is correct and if the name and Social Security number on the card matches your employer's payroll records and year-end Form W-2 (Wage and Tax Statement).

If your Social Security card does not show your correct name or Social Security number, contact your local Social Security office.

If your name and Social Security number, as shown on your Social Security card or your corrected card, match the information shown on your Form W-2, contact your local Social Security office.

If your name and Social Security number on your Social Security card are correct but do not match your employer's records, provide your employer with the correct information as shown on your Social Security card.
