  1. Retain on file a properly completed & current copy of your form MCS-90 financial responsibility endorsement.
  2. Copies of the regulations, forms, interpretations, and manuals are available from a variety of sources. Check theFMCSA website for a current list of suppliers. www.fmcsa.dot.gov/safety-security/eta/index.htm
  3. A copy of your carrier profile can be obtained for $20 from the SAFER website (http://safer.fmcsa.dot.gov) or bycalling 800-832-5660 or 703 280-4001. You can also write: Computing Technologies Inc. P.O. Box 3248, Merrifield,VA 22116-3248. Profile cost if ordered by mail or phone is $27.50.
  4. Ensure that all vehicles are properly marked with your name or trade name and U.S. DOT number. If your vehiclesare also periodically operating for other carriers, they must be marked with that carrier's name and U.S. DOT#.
  5. Drivers may not have Commercial Driver Licenses (CDLs) from more than one state. Ensure that all drivers haveonly one current COL that is not under suspension or revocation. Driver CDLs must also match the correct class ofvehicle driven and have applicable endorsements for double/triple trailer, passenger, tank vehicle and/or hazardousmaterial operation.
  6. Ensure that a trained collection-site person utilizing the proper collection materials and custody and control formsdoes drug-test collection.
  7. Ensure that each drug-test is conducted using the "split-sample" method of collection.
  8. Laboratory must transmit aggregate statistical summary on semi-annual basis
  9. Maintain all required controlled substance testing records including yearly summaries, quarterly summaries, testinformation, test results, records of training etc., as required by 49 CFR Parts 40 and 382 of the FMCSR.
  10. Use only those laboratories certified by the Substance Abuse and Mental Health Services Administration for theanalysis of urine specimens. For a list of certified laboratories check www.drugfreeworkplace.gov or call (301)443-6014.
  11. Ensure that all documents supporting records of duty status (such as toll, fuel repair and other on-the-road expensereceipts, as well as invoices, bills of lading, dispatch records, etc.) are kept on file for at least 6 months.
  12. Obtain from any driver used for the first time (or intermittently) a signed statement showing the total time on-dutyduring the preceding seven (7) days and the time at which the driver was last relieved from duty.
  13. Toll receipts and other on-the-road expense receipts, invoices, bills of lading, dispatch records, and other "supportingdocument" must be kept on file for six (6) months. This requirement also applies to records generated by the use ofowner-operators. You may keep legible photocopies in lieu of originals.
  14. Keep all driver vehicle inspection reports, signed, certified, and reviewed as required on file for at least 90 days.
  15. Require all drivers to prepare a written inspection report for each day a vehicle is operated. Ensure that each report issigned by the driver, certified, and reviewed if defects are reported.
  16. A complete Educational and Technical Assistance package entitled" A MOTOR CARRIER'S GUIDE TOIMPROVING HIGHWAY SAFETY" is available free on the FMCSA website to assist you in complying with the safetyregulations. It contains many forms and documents useful for improving the safety of your operations. Check:www.fmcsa.dot.gov/factsfigs/eta/index.html.
