What documents are required on the day of my interview?
 

The requirements regarding Iraqi police certificates for immigration purposes have changed.   Starting Monday, January 10, 2011, the U.S. Embassy will require an Iraqi police certificate for both Iraqi Nationals and non-Iraqis who lived in Iraq for more than one year.

Required Documents:

  • Two photos each of the petitioner and the beneficiary (Passport size photos)
  • Photocopy of petitioner's U.S. passport or Legal Permanent Resident card
  • Sealed medical examination report from a panel physician.  Follow this link for physicians and fees. DO NOT bring your x-ray.
  • Beneficiary’s current passport (valid for six months beyond the date beneficiary plans to arrive in the U.S.)
  • Photocopy of beneficiary’s current passport
  • All of beneficiary’s previous passports
  • Certificate of personal record (personal information) and a photocopy from Civil Status Department.
  • Original Police Certificate(s) from your country of nationality; your country of residence if you have resided there for six months or more; and a police certificate for any other country in which you have resided for one year or more. This requirement applies only to periods of residence after attaining the age of 16.  Jordanian police certificates are issued by the General Intelligence Directorate.  Certificates issued by the Jordanian courts are not acceptable.
  • Form DS-230, Parts I & II (All questions on the form must be answered)
  • Original or certified copy (by issuing authority) of your birth certificate plus a photocopy and English translation.
  • Original or certified copy of your marriage certificate issued by Jordanian civil status Department plus a photocopy and English translation.
  • Original or certified copy of your marriage contract (katb al-kitaab) plus a photocopy and English translation.  Contracts that list divorced spouses as “single” will not be accepted and must be corrected by the Sharia Court.
  • Original or certified copy (by issuing authority) of all divorce certificate(s), if applicable, plus a photocopy and English translation.
  • Original or certified copy (by issuing authority) of spouse’s death certificate, if applicable, plus a photocopy and English translation.
  • Original, signed affidavit(s) of support with the three most recent years’ income taxes and W-2 statements from the petitioner and any joint sponsor.
  • Applicants with Iraqi passports must also provide the original nationality certificate and personal ID card
  • Original or certified copy (by issuing authority) of applicant’s court documents related to all previous arrests, even if the case was dismissed, plus a photocopy.
  • Original or certified copy (by issuing authority) of documents related to all applicant’s previous immigration proceedings or removal hearings, plus a photocopy.
