SponsorshipTo initiate the Fort Benning Sponsorship Program it is essential that Soldiers complete the DA Form 5434 (Sponsorship Program Counseling and Information Sheet) during their reassignment processing at their last duty station. The DA Form 5434 advises the command of any special needs and support mechanisms that the Total Army Sponsorship Program must muster to help the Soldier and his family. The completed DA Form 5434 should be sent to: Chief, Personnel Operations Branch, Military Personnel Division, ATTN: IMSE-BEN-HRM-M, Enlisted Strength Management Section, Fort Benning, GA 31905. All military personnel are required to sign-in at Installation Replacement Operations, Building 2607, Soldiers' Plaza, 706-545-4577, for in-processing. Request a sponsor: Fill out this form, DA5434, then save it to your computer as "LASTNAME_DA5434" and Click here to email. The Newcomers Orientation will help you feel right at home. ID Cards, Vehicle & Weapons RegistrationIdentification CardsThere are two different identification card sections servicing the Fort Benning area. The main ID card section provides services for all permanent party personnel (with the exception of Soldiers with the 3HBCT, 3rd Infantry Division), Soldiers attending Officer Candidate School, Ranger School, Airborne School, Infantry officer advanced or basic course, military retiree’s, disabled veterans and all Family members. The section operates only by appointments. It is located in Building 2638 in Soldiers’ Plaza and it is open Monday through Friday, 8 a.m. to 4:30 p.m. The section is closed all federal and training holidays and may close without advance notice to support contingency operations. MACH providers care for more than 48,000 TRICARE-enrolled beneficiaries - approximately 25 inpatients and 1,800 outpatient visits daily in one of eight primary-care clinics both at Fort Benning and at remote locations in Georgia and Florida. Construction on a new 745,000 square foot, state-of-the-art facility is expected to resume shortly and a new combined medical and dental clinic will open in January 2011 to support Armor School Soldiers. Active-duty Soldiers will complete TRICARE enrollment during inprocessing at Soldiers Plaza. Sponsors need to have a DA Form 5888 completed by the child’s primary care manager (PCM). The EFMP office is next door to the Peds Too Clinic in the hospital’s first-floor clinic wing. For information, call 544-1939. Pregnant PatientsPregnant patients are seen in either the Family Medical Home or OB/GYN Clinic once enrolled for care at MACH. Patients may participate in the hospital’s new Centering Pregnancy program - care with a provider and a midwife. For information, call 544-3201. School Screenings, Physicals and ImmunizationsStudents enrolling in Georgia and Alabama schools for the first time require certain screenings. Georgia requires a Certificate of Immunization (Form 3231 Revised) and a completed Vision, Hearing and Dental Certificate (DHR Form 3300). Alabama requires a Certificate of Immunization (IMM-50). Call 544-2273 for appointments for these enrollment screenings as well as appointments for school and sports physicals. The military command, school board and parents provide wonderful community support to us and we feel fortunate to serve the families of our military sponsors. We have a total of 7 schools (6 elementary and 1 middle) which serve the families living on the Ft. Benning installation. Children attend our schools from Pre-K through 8th grade.
