Down load the registration form with DTS Profile, fill out and mail it to

The below form can be saved to your desktop, completed, and e-mail to the below e-mail address. When saving the form, Please save it with the following naming format, example; Smith-120th PSB

**Only Family Members and Designated Individuals who qualify for DTS Travel Reimbursement are required to complete the bank account section of the form due to The Joint Federal Travel Regulations requirements. Refer to the Family Member LOI mailer for reimbursement qualification requirements.
