Have an appointment you need to cancel? Now you can e-mail us to cancel your appointment! Click the link and PLEASE BE SURE TO INCLUDE THE FOLLOWING:
- Full name of patient with the appointment
- Where your appointment is (i.e. Family Practice, Dermatology, Optometry etc.)
- Date and time of appointment to be cancelled
- Contact number

A representative will call to confirm the cancellation at which time you'll have an opportunity to reschedule your appointment.
