When arriving for your scheduled appointment please remember to bring your appointment card. This card should contain your appointment date and time, clinic location, and a corresponding lab number if your provider ordered labs for your visit. Remembering to present this card at your visit will remind us of your provider’s intentions and enable us to provide you with faster service.
