Transitional Living Center (TLC)

About the Program

The goal of the Transitional Living Center (TLC) is to provide veterans the opportunity to achieve optimal function in order to maintain or improve the quality of life. Activities are individualized and planned within a collaborative framework of therapeutic programs, thus, restoring veterans to their maximum health potential, which may include palliative/end of life care.

We have a 32 bed occupancy at the Transitional Living Center where we provide 3 different types of care as follows:

1. Respite care - for caregivers who need a break in order to maintain the veteran in the home
2. Rehabilitation or Skilled Nursing - for those who need IV antibiotics or a little rehab following a stroke, etc.
3. Long term care - this is only for a select few veterans who are service connected and is in line with congressional mandates.

Residents typically share a room and are served meals in a central dining area. Residents will have the opportunity to be involved in activities that provide mental, physical, and social stimulation.

HPC - Hospice Palliative Care

Hospice provides comprehensive medical and supportive services across a variety of settings and is based on the idea that dying is a part of the normal life cycle. You will receive care and support that is tailored to meet your needs and wishes at end of life. Palliative care is comfort care which strives to prevent and relieve suffering and to support the best possible quality of life for patients and their families, regardless of the stage of the disease or the need for other therapies. Currently we have a 8 bed inpatient hospice unit located in the TLC for Veterans who are unable to maintain at home. We also have a Palliative Care Consult Team (PCCT) that covers the entire medical center and provides consultation and coordination of care/services. All veterans are eligible for hospice services that are deemed hospice appropriate by a VA physician. Hospice and Palliative Care cannot add days to your life, but can add life to your remaining days.
