Exceptional Family Member Program (EFMP) Reynolds Army Community Hospital
AR 608-75 mandates that all Soldiers who have a Family member with an identified medical and/or educational condition must enroll in the Exceptional Family Member Program (EFMP). Soldiers are also responsible to update EFMP enrollment every three years; however, updates can also be made anytime the condition changes.

To enroll and/or update EFMP medical enrollment: • Complete personal information on the DD Form 2792, EFMP Medical Summary. • Contact the Reynolds Army Community Hospital (RACH) EFMP Special Needs Advisor, Irene Reid at (580) 558-3460 for updates, dis-enrollments or to request a copy of your EFMP Summary Report. • Make an appointment with your Family member’s Primary Care Manager (PCM) and bring your completed DD Form 2792 and EFMP Summary Report to the appointment. • Following appointment with PCM, take the completed DD 2792 to the RACH EFMP Special Needs Advisor located at the East entrance of the hospital room 1D120. She will check for completeness and send paperwork to Department of Army (DA) to update EFMP medical enrollment.

To enroll and/or update educational enrollment for children on Individual Educational Plan (IEP): • Complete personal information on the DD Form 2792-1, EFMP Special Education/Early Intervention Summary. • Take DD Form 2792-1 to your child’s Special Education Teacher to complete items 3-6. • Once DD Form 2792-1 is complete, take it and a copy of your child’s IEP to the RACH EFMP Special Needs Advisor located at the East entrance of the hospital room 1D120. She will check for completeness and send paperwork to DA to update EFMP educational enrollment.

RACH EFMP is here to assist you and your Family member needs to be dis-enrolled from the EFMP i.e. the condition no longer exists, child no longer on IEP, divorce, etc., please contact RACH EFMP for additional information. See map for directions the RACH EFMP.

To be cleared for Overseas Travel, you will need the following:
1. Call 580-558-2000 for an appointment for each family member traveling with sponsor.
2. Children: Current physical (within the last year) and shot records.
3. Women (spouse): Current well woman’s exam (within the last year) and shot records.
4. Men (spouse): Current physical (within the last year) and shot records.
5. Exceptional Family Deployment Screening Sheet, DA Form 5888, Soldier should complete #1-#7 and #8 is signed by the Soldier’s Personnel Section. Part B may only be signed by an authorized EFMP Physician.
6. Dependents outpatient records.
7. Children under 6 years of age need to be screened by EFMP Physician.

To be cleared for Long Distance Overseas Travel, you will need the following:
1. Children (spouse): Current physical (within the last year) and shot records.
2. Women (spouse): Current well woman’s exam (within the last year) and shot records.
3. Men (spouse): Current physical (within the last year) and shot records.
4. Exceptional Family Deployment Screening Sheet, DA Form 5888, Soldier should complete #1-#7 and #8 is signed by the Soldier’s Personnel Section. Part B may only be signed by an authorized EFMP Physician.
5. Screening of Family Members In Remote Oconus Areas, DA Form 5888-1, filled out and signed by treating physician.
6. Exceptional Family Deployment Program Screening Questionnaire, DA Form 7246, filled out and signed by treating physician.
7. Pediatric Development Questionnaire complete for each child under the age of 6 years.

The Exceptional Family Member Program (EFMP) is a mandatory enrollment program, established per Army Regulation 608-75, November 2006, that works with other military and civilian agencies to provide comprehensive and coordinated medical, educational, housing, community support and personnel services to Families with special needs. Soldiers enroll in the program when they have a Family member with a physical, emotional, developmental or intellectual disorder requiring specialized services so their needs can be considered in the military personnel assignment process.

All Family members must be screened and enrolled (if eligible) when the Soldier is on assignment instructions to an OCONUS area for which command sponsorship/ Family member travel is authorized, and the Soldier elects to serve the accompanied tour. This screening consists of medical records review for all Family members and developmental screening for all children 72 months of age and younger.

The Exceptional Family Member Program Office at Reynolds Army Community Hospital is located on the first floor at the North entrance in room #1B140. Come by to pickup a Summary Report or for information regarding enrollment criteria. The EFMP Office provides information and administrative assistance for family members enrolling in the Exceptional Family Member Program such as updating, enrolling and disenrolling.

Routine updates are scheduled with your Primary Care Manager or treating physician.
