Home Based Primary Care (HBPC) Mission/Vision:To honor America’s Veterans by providing exceptional health care in the home setting that supports the Veteran’s changing needs and preferences throughout the course of their life. Scope Service:The goal of HBPC is to provide primary care in the home. The program helps Veterans remain independent and have a better quality of life. HBPC provides care for 150 Veterans. HBPC Veteran Services:• Nurse case management. • Home visits from HBPC staff. • Telephone contact. • Help with medications. • Assistance with health issues. • Health and self care education. • Family and caregiver support. HBPC Staff Includes: • Nurses • Medical Providers • Social Workers • Dietitians • Physical Therapist • Psychologist • Pharmacist • Program Support |
