Appointments scheduled between 0720 and 1420 weekdays, subject to closure during Holidays, manpower shortages, or technical issues. Also, Closed from 1130 - 1230 (Admin and Lunch)
APPOINTMENTS
Appointments scheduled online - appointment times slots are between 0720 and 1440 daily; one appointment time for each family. No walk-ins will be accepted for non-emergency service.
EMERGENT ISSUES
Limited service for Pin Resets and Lost ID from 0720 to 0900 on operational days: initial consultation requires no appointment; customer priority based on check-in time; service cannot interfere with regularly scheduled appointments beginning at 0900.
LOST/STOLEN ID
Must have a signed letter from their command stating the circumstances under which ID has been lost/stolen prior to receiving a new ID. The letter must be signed by Company Commander or Division Officer – see MCBH BaseO 5512.5A for format and details.

WHAT TO BRING
Documents must be original or certified copies by the issuing agency. All customers (21 & older must have two (unexpired) forms of ID). One must be a state or federal gov't issued photo ID. Acceptable forms of ID: http://www.uscis.gov/files/form/i-9.pdf
DEPENDENT ID CARDS
Sponsor (in person) must be present to sign DD Form 1172-2. If unavailable, the dependent must have a previously signed 1172-2 (within 90 days of signature) signed by their sponsor and the VO/SSM or a POA(Power Of Attorney). Refer to the RSS link above.
ADDING A SPOUSE INTO DEERS
Sponsor (in person) will present the marriage certificate, birth certificate, social security card, & valid photo ID. All documents original government issued, or certified true copies by the issuing agency.
ADDING CHILDREN INTO DEERS
Sponsor will present original birth certificate & social security card (marriage certificate for stepchild); (Court order or VAP form for out of wedlock).
FULL TIME COLLEGE STUDENTS
College students must bring a letter from their school registrar's office verifying full time enrollment in an accredited college in pursuit of an AA/AS or higher to have dependant benefits extended to age 23.
RENEWING ID
Not authorized unless the ID card is within 90 days of expiration (unless expiration is during TAD, orders required) with documentation i.e. extension paperwork.
