Proof of Health Care/Medical Benefit
This article provides information relating to requesting a TRICARE eligibility letter from Defense Manpower Data Center (DMDC).
You or your family member may receive proof of health care/TRICARE medical benefit by contacting the DMDC at 1-800- 538-9552. DMDC requires that you send a signed, written request by mail or fax to: DMDC Support Office, 400 Gigling Road, Seaside CA 93955, fax (831) 655-8317.
The following information is needed before your request can be processed by DMDC:
- Military member/sponsor name and Social Security Number (SSN)
- All family member names
- Name and address of family member the request should be sent to
- Signature of the requestor
If you or your family member is currently eligible for TRICARE, an "Eligibility Letter" will be sent by DMDC. If you are a former TRICARE beneficiary, requiring proof of health care/medical benefit, a Certificate of Creditable Coverage (CoCC) will be sent by DMDC. The CoCC is proof of previous health care/medical benefit for the period of time listed on the certificate.
