VA Social Work Service and Geriatrics and Extended Care (GEC)

Social Workers at the Providence VAMC can be found throughout a variety of programs providing a range of services to  veterans .  They are licensed master's level clinicians who can assist with case management, care coordination, individual and group psychotherapy, psychosocial assessment, home health care coordination, community education, caregiver supports, and end-of-life and advance care planning.

Our Social Workers assist homeless veterans, nephrology patients, returning veterans and their caregivers,  primary care patients, geriatric patients, and those struggling with mental illness.  Social workers are available    to provide education on Advance Directives, home health programs, and community resources. Our geriatric extended care program provides home-based primary care services, home tele-health, adult day program, respite services, and nursing home placement.
