DS-11 Application Form

IMPORTANT: The DS Form 11 must be signed in front of a Consular Officer at the US Embassy. In order to sign the DS Form 11 in front of a Consular Officer, one must make an appointment to visit the US Embassy.

This form is for applicants who:

• Cannot present their last passport (lost or stolen);

• Have never been issued a US passport;

• Hold a passport issued more than fifteen years ago;

• Hold a five year passport.

Special instructions for passports for children under 16.

If one of the above apply to you, please come personally to the Embassy with one or more of the following documents:

• Original U.S. birth certificate or original U.S. Consular Report of Birth Abroad;
• One of your previous U.S. Passports;
• Certificate of Naturalization;
• Current Photo ID.

You will also need to bring:

1. Photograph:

   • One 2" x 2" (5 cm x 5 cm) color photograph taken within the past 6 months; full face, frontal view against a plain white or off-white background; size of head should be between 1 inch and 1 3/8 inches (2.5 cm and 3.5 cm) from the bottom of the chin to the top of the head. Danish passport size photos are not acceptable.
     
     • Examples of "good" passport photos.

2. Fee:

   • Over 16 years old: US$ 135.00/DKr 877.50.

   • Under 16 years old: US$ 105.00/DKr 682.50

   • Fee can be paid by cash, credit card or Danish kroner checks.  Checks should be made out to “U.S. Embassy”.
3. If you wish your passport to be sent to you please provide us with a self addressed stamped envelope  with 92 Dkr in stamps. Please note that we cannot mail documents outside of Denmark.  Applicants from outside of Denmark will be required to return to pick up their passports or provide a Danish mailing address.

The Passport and Citizenship Unit is open to the public between 9:00 and 12:00 noon daily, except Wednesdays, American and Danish holidays.
