Children's Passport
 

IMPORTANT: Passport Applications for Minors must be signed in front of a Consular Officer at the Embassy. To visit the Embassy, one must make an appointment. Only one appointment is needed per family.

1) Use Form DS-11 (Do not sign the form) Child and parents must be present.
2) Present proof of U.S. citizenship:
  • Original U.S. birth certificate, or
  • Previous five-year U.S. passport, or
  • Original Consular Report of Birth Abroad (Form FS-240), or
  • Original Certification of Birth Abroad (Form DS-1350), or
  • Original Certificate of Citizenship or Naturalization.
3) Submit Evidence of Child's Relationship to Parents/Guardians:
  • Original Certified U.S. birth certificate (with parents' names); or
  • Original Foreign birth certificate (with parents' names and translations, if necessary); or
  • Original Report of Birth Abroad (with parents' names); or
  • Certification of Birth Abroad (with parents' names)
  • Original Adoption Decree (with adopting parents' names); or
  • Original Court Order establishing custody or guardianship.
4) Each parent or guardian must submit proof of identity:
  • Passport or
  • Driver's license
5) Submit Parental Application Permission Documentation:
  • Both parents must appear together and sign; or
  • One parent appears, signs, and submits second parent's written notarized statement of consent authorizing passport issuance for the child DS-3053(PDF)

As of November 1, 2004, this statement of consent from the non-applying parent must be notarized. The statement must indicate that parent´s unequivocal consent to support issuance. Statements notarized by foreign (i.e. non-U.S.) officials will be accepted but must be in English. Parents may use form DS-3053(PDF)or an affidavit of their own. or:

  • One parent appears, signs, and submits evidence of sole custody authority
  • Child's original U.S. or foreign birth certificate listing only applying parent; or
  • Original Consular Report of Birth Abroad; or
  • Original Certification of Birth Abroad, listing only applying parent; or
  • Original Court order granting sole custody to the applying parent; or
  • Original Adoption decree listing sole adopting parent; or
  • Original Court order specifically permitting applying parent's or guardian's travel with the child; or
  • Original Judicial declaration of incompetence of non-applying parent; or
  • Original Death certificate of non-applying parent.

6) Fee: US$ 105.00/DKr 682.50.  For payment made at the Embassy, we accept cash, credit card or Danish kroner checks.  Checks should be made out to “U.S. Embassy”.

We strongly encourage both parents to appear at the Embassy whenever possible. The two-parent consent rule has been strengthened as a means of preventing international child abduction.

If you live in Jutland or Fyn, you may make an appointment to apply for a passport during the fall or spring consular visits, in lieu of applying at the Embassy. If you are registered with the Embassy and have supplied your e-mail address you will receive notification of these visits

Passport for Minors

  • New U.S. Passport Requirement
    (Effective February 1, 2008)

    All Minors Must Appear in Person.

    To enhance the accurate identification of passport applicants and aid in the prevention of international child abduction and trafficking, the Department of State's Bureau of Consular Affairs has begun requiring the personal appearance of all minors applying for U.S. passports.

    This applies to all regular, official and diplomatic passports for children under the age of 16, even if the child has previously been issued a passport. Prior to the introduction of this requirement, parents were not routinely required to bring a child under the age of 16 with them when they applied on the child's behalf. Now, when applying for a passport on behalf of a minor under the age of 16, parents are required to have their child present and submit documentation of parental relationship and consent, as well as comply with all other documentation requirements for the issuance of a passport.

    The personal appearance requirement for all minors is a further step towards ensuring the integrity of the passport application process. This change will help to verify the identity of minor applicants and aid in the prevention of international child abduction and trafficking.
