I-94/I-94W Departure Record Card
 

I did not turn in my I-94/I94W on my last trip to the U.S.  What should I do?

When you depart the United States you must return the white I-94 or green I-94W departure record card to the airline or ship representative.  If you do not you may be recorded as overstaying your admission.  This may cause you to be seriously delayed or even denied entry by the U.S. Bureau of Customs and Border Patrol (CBP) the next time you travel to the U.S.

If you forgot to return your I-94 or I-94W when you left the United States you should follow the steps below to correct your record and prevent problems when you travel to the United again:

Send the I-94 or I-94W and documentation that proves you left the United States to the U.S. Bureau of Citizenship and Border Patrol (CBP) to:

CBP will consider different types of documentation including:

  • Original boarding passes you used to depart the U.S. or another country (e.g., Canada), to return home;
  • Photocopies of entry or departure stamps in your passport showing that you entered another country after departing the United States (you should copy all passport pages that are not completely blank, and include the biographical page containing your photograph); and/or
  • Photocopies of other evidence, such as:
  • Dated pay slips or vouchers from your employer indicating you worked in another country after departing the United States,
  • Dated bank transactions showing you were in another country after leaving the United States,
  • School records indicating you were in another country after you left the United States, and/or
  • Dated credit card receipts, showing your name (but not the credit card number) for purchases made after you left the United States.

To help CBP understand the situation and correct your records quickly, please include an explanation in English. Your statement will not be accepted without the type of supporting evidence mentioned above.  Be sure to send original or legible copies when possible. If you send original documents, you should keep copies. CBP cannot return originals after processing.

Also, take a copy of what you send to CBP with you the next time you go to the United States in case the CBP officer has questions.  This will also allow CBP to make the corrections if the London, Kentucky office has not yet done so.  Do not mail your I-94 or supporting evidence to a United States Consulate or Embassy or to any other CBP office in the United States.  CBP can only make corrections at the London, Kentucky facility.
