Military CAC Appointments

Camp Butler/Foster ID Card Center utilizes an appointment based system for CAC/ID card issuance. This service was implemented to assist in better servicing you our customer by reducing the wait time prior to receiving a new ID card. To ensure each customer being assisted has the correct documentation and increase customer satisfaction, the below documents inform each person of what is needed to receive a CAC/ID Card. Please read through the following information which is case specific to make certain all requirements are met prior to your visit to IPAC. If you have any additional questions contact the ID Card Center prior to your appointment at: 645-4038/4039.
Original Promotion Warrant (signed by authorized signer) or Frocking Letter. If you are an Officer recently promoted and have not yet received your Promotion Warrant, we can accept a letter of Promotion reflecting the date of promotion signed by the Commanding Officer or Unit Adjutant “by dir”. If unable to obtain any of these documents then it will be necessary to wait for DEERS to reflect the rank change.


  • Two original forms of ID, one Primary and one Secondary
  • Lost ID Card statement signed by the Commanding Officer or Company GySgt for E5 and below.
  • A Minor Offense Report from PMO Family member lost ID Card:
  • Two original forms of ID one Primary and one Secondary no exceptions.
  • Sponsor present or Original Power Of Attorney or an original DD 1172 form reflecting sponsors’ signature in block 90. If the DD form 1172 is not
    signed in the presence of a Verifying Official then it must be notarized.
  • A Minor Offense Report from PMO
  • Two original forms of Identification, one Primary and one Secondary
  • Original marriage certificate, translated to English if necessary from an authorized translation office, reflecting the translation seal.
  • Original birth certificate, we are unable to accept certified true copies unless they are from the State of issuance.
  • Original Social Security card/ITIN or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)
  • Sponsor present, or Original Power of Attorney
  • If you are unable to obtain original documentation, an option would be to send a Special Power of Attorney for DEERS enrollment to your spouse.

  • Original birth certificate, or Hospital proof of birth, translated to English if necessary from an authorized translation office, reflecting the translation seal.
  • Original Social Security card or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)
  • Children 21-23 years require, a letter from the School Registrar on letterhead certifying enrollment in a full time accredited institution of higher learning leading to an Associate’s degree or higher. In lieu of the letter from the registrar a National Clearing House letter maybe utilized.
  • Sponsor present, or original Power of Attorney, or DD 1172 signed by Sponsor in the presence of a Verifying Official at an ID Card Center.
  • Initial enrollment for a step-child requires, the original marriage certificate reflecting the mothers name and the child must reside with the sponsor. IF child is 21-23 and a Full time student:
  • Students 21-23 years, a letter from School Registrar certifying enrollment in a full time accredited institution of higher learning. In lieu of the letter from the registrar a National Clearing House letter maybe utilized.
  • Two original forms of Identification, one Primary and one Secondary IF child is a Ward:
  • Original birth certificate
  • Original Social Security card or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)
  • Child’s Dependency Affidavit from HQMC
  • Active duty personnel shall complete a DD Form 137-7, Dependency Statement-Ward of a Court in accordance with MCO P1751.3 when applying in behalf of a dependent ward, have it notarized and forward to HQMC (MRP-1).
  • Court Order from a court of competent jurisdiction in the United States court placing the child in members custody for at least 12 consecutive months
  • Dependency determination from HQMC is needed each time for ID card renewal
  • For initial enrollment sponsor must visit the parent service ID Card Center (ex: Air force, Kadena; Navy, PSD, etc)
  • Temporary ID cards are not authorized pending HQMC approval.
  • Original birth certificate
  • Original Final Adoption Decree.
  • Original Social Security Card or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)
  • Sponsor present, or original Power of Attorney, or DD 1172 signed by Sponsor in the presence of a Verifying Official at an ID Card Center.
  • If the child is adopted overseas please Contact Base Legal at 645-7144 for further requirements
  • Sponsor or spouse’s original birth certificate reflecting the parents’ name
  • Two original forms of Identification, one Primary and one Secondary
  • Original Social Security card or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)
  • HQMC dependency approval letter
  • For initial enrollment sponsor must visit the parent service ID Card Center (ex: Air force, Kadena; Navy, PSD, etc)
  • Female member must present original birth certificate, or Hospital proof of birth, translated to English if necessary reflecting the translation seal
  • Male member must present original birth certificate, or Hospital proof of birth, translated to English if necessary reflecting the translation seal, and court order establishing paternity.
  • Original Social Security card or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)

OR
  • Male member must present original birth certificate, or Hospital proof of birth, translated to English if necessary reflecting the translation seal, and approved dependency and residency determination (reflecting over 50% support).
  • Original Social Security card or documents in lieu of a SS card. (Ex. Tax document reflecting SSN, Pay stub, or Checking Acct Statement reflecting SSN)
  • Children 21-23 years require, a dependency determination (reflecting over 50% support), a letter from the School Registrar on letterhead certifying enrollment in a full time accredited institution of higher learning. Provide HQMC with a notarized DD Form 137-6 Dependency Statement-fulltime student 21-23
  • Sponsor present, or original Power of Attorney, or DD 1172 signed by Sponsor in the presence of a Verifying Official at an ID Card Center.
  • For initial enrollment sponsor must visit the parent service ID Card Center (ex: Air force, Kadena; Navy, PSD, etc)
  • All personnel who require an initial or renewal ID card must have two original forms of identification. Documents that properly establish identity are broken into two groups: Primary proof of identity and Secondary proof of identity. Primary documents include valid State or Federal government issued picture identification (ID). One primary document must be captured. After a Primary proof of Identity is chosen, a second document must be presented that comes from either the Primary or Secondary group, so long as it is different from the one initially captured.

    Group A/Primary
    • U.S. Military Card
    • Military dependent ID card
    • Driver's license or ID card issued by a state or outlying possession of the U.S. provided it contains a photograph
    • U.S. Passport

    • Sponsor present or Original Power Of Attorney or an original DD 1172 form reflecting sponsors’ signature in block 90. If the DD form 1172 is not signed in the presence of a Verifying Official then it must be notarized.

    • Two original forms of ID, one Primary and one Secondary
    • Current contract showing contract number, Period of Performance, and family members names
    • A Minor Offense Report from PMO Family member lost ID Card:
    • Two original forms of ID one Primary and one Secondary (exception: Children under the age of nine years, require one valid ID)
    • Current contract showing contract number, Period of Performance, and family members names
    • Sponsor present or Original Power Of Attorney or an original DD 1172 form reflecting sponsors’ signature in block 90. If the DD form 1172 is not signed in the presence of a Verifying Official then it must be notarized.
    • A Minor Offense Report form PMO
