About the Program

Social Work is an integral component in the delivery of medical and behavioral service within the Cincinnati VA Medical Center.

Programs include but are not limited to--

  1. Medical/behavioral services
  2. Community outreach
  3. Homeless services
  4. Domiciliary
  5. Recovery programs
  6. Specialized programs, OEF/OIF/OND (Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn), SCI (Spinal Cord Injury), TBI (Traumatic Brain Injury), VIST (Visual Impairment Services Team), PTSD (Post-Traumatic Stress Disorder), VTAC (Veterans Activity Treatment Center)
  7. Hospice and palliative care services and support
  8. Contract home health and nursing home services
  9. Home-based primary care
  10. Geriatric care
  11. Caregiver support care

Social workers provide an array of individual and family focused services to include; discharge planning, counseling, guardianship consultation, advance directive and living will education, a continuum of mental health services, emergency evaluations, and referrals to VA and community resources.

Counseling services may include individual, couple, family, and group forums. Counseling is offered to provide:

  • problem specific therapy
  • supportive counseling; typically adjustment to life stress and/or adjustment to illness
  • maintenance of chronic mental health illness
  • intensive case management

Discharge Planning is designed to address current and expected medical/behavioral needs. As a member of the multidisciplinary medical team social work will coordinate, arrange, and make necessary referrals to facilitate the least restrictive home and nursing home care needed to maintain an acceptable quality of life.

Caregivers provide a valuable service for Veterans.  They are "partners" with VA in providing excellent health care.  Caregivers allow Veterans to remain in their own home.  They also support Veterans who are hospitalized or living outside their home.  Cincinnati's VA Caregiver Support: Elizabeth "Liz" Grader, 513-889-8207 and Mollie Snyder, 513-889-8213.  The toll-free number for VA National Caregiver Support Line is 855-260-3274

Social Workers are assigned to specific areas and can be reached at the appropriate clinic, primary care, and emergency room department.
