I Want to Apply for Social Security Disability Benefits
Before You Apply:
- Read the Adult Disability Starter Kit fact sheet to understand the application process and what you should know before you apply for Social Security Disability Benefits.
- Use the Social Security Administration’s Benefits Eligibility Screening Tool (BEST) to learn if you may be eligible for benefits.
When You Apply:
- If the results from BEST show that you might be eligible for Social Security benefits, you can apply online. Before you begin, review the Adult Disability Checklist.
- You will need to fill out the Disability Benefit Application and Adult Disability Report; then, complete the Authorization to Disclose Information Form (SSA-827). (This form can be completed electronically as part of the Adult Disability Report.)
- If you don't want to apply online or if you have questions about applying, call SSA toll-free at 1-800-772-1213 (1-800-325-0778 TTY) Monday through Friday from 7:00 a.m. - 7:00 p.m. EST to make an appointment to apply for benefits.
- You can also contact your local Social Security office for assistance.
