Exceptional Family Member Program (EFMP)

What Forms I need:

Overseas Screening - DA Form 7246, and Form 5888, Proof of Physical Exam from health care provider

EFMP Enrollment - Form 2792 (Medical) and Form 2792-1 (Educational/IEP)

Criteria for Enrollment:

All Soldiers are required to have a face to face appointment with the EFMP Case Coordinator located in the Pediatric clinic of Kenner for coordination for services, schools and housing.

All children with special needs are required to be accessed by the Special Needs Resource Team before they are allowed to attend Child and Youth Services.

Family members are screened for enrollment when:

  • Traveling to a new duty station with sponsor and they are eligible for health care and/or education at government expense
  • They require medical care above a level provided by a family practitioner in an outpatient setting.
  • Family member has serious chronic medical problems, physical disabilities and /or mental issues.
  • Special education services are required from ages 3-21 years of age, to include speech, occupational and physical therapy.
  • Inpatient/outpatient mental health services within the past five years.
Examples of conditions requiring enrollment:
  • Asthma, Sickle Cell disease and Insulin dependent Diabetes
  • Attention Deficit Hyperactivity Disorder-requiring medication
  • Treatment by a mental health care provider and/or counsellor.
  • Intensive follow-up support by specialist: High risk newborns, cancer patients, leukemia, etc.

EFMP conducts all overseas screenings for family members.  Enrollment to Kenner Army Health Clinic is not required.
