About the Program
The Extended Care and Rehabilitation Program at Fargo VA Healthcare System is composed of three areas —
- Physical Medicine and Rehabilitation
- Community Living Center
- Home and Community Care
Services Offered
Physical Medicine and Rehabilitation (PM&R)
PM&R services include —
- occupational therapy
- physical therapy
- speech therapy
- recreational therapy
- specialized services to veterans in need of rehabilitation following amputation, stroke, traumatic brain injury, orthopedic surgery and spinal cord injury
Community Living Center (CLC)
The CLC is a long term care unit, which maintains an average length of stay of 30 days. Most patients require extended rehabilitation and care following surgery and/or lengthy hospitalizations before returning to independent living. End of life/palliative and respite care for caregivers is also available.
Home and Community Care (HCC)
HCC consists of programs that manage care in veterans homes and community settings. Services include —
- home-based primary care
- adult day health care
- contract community nursing home care
- skilled and unskilled home care
- home hospice care
- home respite
