Using Technology to work with Veterans – Miami VA Telecare Urban Home Health Program

Miami VA Telehealth Program Have Miami VA Healthcare Professionals monitor your health remotely using secure technology. The T-CARE program’s mission is to provide timely, high quality health care, individualized to meet your needs. By using remote technology to monitor basic information, you can set goals and improve your health all with the support of a Miami VA provider.

Following a home or office visit to set up and train you on using the in-home messaging device, you will be monitored by your T-CARE team on a regular basis. Remember, the in -home messaging device is not an emergency service. If you have an emergency, please call 911.

Why Telehealth?

This technology will allow theT-CARE team:

  • To monitor your health condition.
  • To provide support and education.
  • To determine when extra attention is needed.

Who is eligible?

You may participate if you:

  • Reside in Dade, Broward, or Monroe County;
  • Have a chronic disease such as diabetes, hypertension, heart disease or lung disease;
  • Are willing to accept and use technology in your home.

What is a Care Coordinator?

A Care Coordinator may be a Registered Nurse (RN) or an Advanced Registered Nurse Practitioner (ARNP). Your care coordinator is a professional that helps to meet your needs in collaboration with your primary care provider. This can include information on your medical condition and information on VA and community services. Your care coordinator will make every effort to assess your needs and coordinate your care with your primary care provider or with other services available.
