Thank you for choosing this facility for your training needs This web page has been created to ensure that all paperwork is completed prior to your rotation cycle. You should start this process at least two weeks prior to your rotation. (See Table for Required Paper Work and Mandatory Time Frame for Submission)

Mandatory Training for Trainees" Web Course  -  Must Be Completed By ALL Residents, Students and Trainees.

On-line training should be completed at least 2 weeks prior to the start of the rotation to avoid rescheduling the rotation. To complete the course, go to the Talent Management System (TMS) and follow the step by step instructions to create a TMS account. This is a yearly requirement.  Submit all paperwork to your respective department you are rotating in.  If you do not know the department you are rotating in, contact Simon Maxwell via e-mail at Simon.MaxwellJR@va.gov.  Computer codes will be given to you by the assigned service. If you have any questions or concerns contact one of the individuals in the Point of Contact section below.

Forms to complete, print, sign and submit at least 2 weeks prior to the start of your rotation.  Please indicate where you will be doing your rotation and the expected start and end date in a fax with cover sheet, or by phone to the department/program you will be rotating in.

Students & Trainees  WOC Appointment Request Two weeks prior to the start of your rotation:  Submit to the department you will be rotating in.  Complete the top portion of this form.

Two weeks prior to start of your rotation:   Submit to the department you will be rotating in.  Complete blocks 1-17.
Residents, Students & Trainees VA 0711  Two weeks prior to start of your rotation:   Submit to the department you will be rotating in.  You only need to complete the top part of the first page (Section 1, blocks 1-9, Section 3, Part A, Blocks 3-9, and Section 4, Part B, blocks 1-3).
Residents, Students & Trainees Mandatory Training for Trainees (MTT) Certificate

Once you have submitted your paperwork, the process will begin to give you access codes to the computer system. Computer codes will be given to you by the assigned service. Residents, Students & Trainees Accepted Forms of Identification 

When you go to the Police Unit you will be finger printed and receive an ID badge.  Two acceptable forms of identification will be required

Residents Resident Appointment Letter  Complete the top portion with your name, rotation dates, the department you are rotating in, and the responsible VA staff person in that department (if known) that you are training with and fill out the bottom portion of the form.
Residents SF-61  Two weeks prior to start of rotation:  Residents must complete and submit an SF-61 to the department you will be rotating in.  Make sure to complete the following blanks: position to which appointed, date appointed, department, your name above the oath of office, and your signature of appointee.
Residents NPI# * Residents must apply for an NPI# before starting their rotation at the Detroit VAMC.
