                  PROCEDURE FOR OBTAINING A HANDICAP PARKING CARD.

 Please read the attached informational memorandum and letter of  medical eligibility for handicap

 parking. Your medical provider must complete and sign the Letter of Medical Eligibility. 

 Please take the items listed below to your local Vehicle Registration Office for processing:

  ·  Two color passport size photos.

   ·  The completed form signed by your physician/medical provider.

 Upon submission of your application paperwork you will be issued a Temporary Handicap Parking Authorization Card.  The new temp card will be utilized only for the application processing time and expires upon receipt of the formal handicap card when picked up at the servicing Field Registration Office or 5 working days from the date the standard card was mailed.

 Only one Parking Card will be issued per person, and may be transferred between vehicles transporting that handicapped person. The handicap parking Card is valid only for the dates specified by your physician/medical provider on the attached form and may only be used at U.S. Military Facilities in the European Medical Command footprint.  When completed, your new Handicap Parking Card will be mailed to your local vehicle registration office for pick-up.
