Lost or Stolen Passports
 

The Consulate General accepts applications to replace passports that have been lost, stolen, or mutilated (damaged). Unless you have a need to travel within the next 10 business days (and can present evidence of this need to travel), you will still need to make an appointment to apply for a replacement passport. Once you have made an appointment, you will need to:

An applicant without a passport, due to loss or theft, must come to the Consulate General in Barcelona between 09:00 AM to 13:00 PM and:

  1. Appear in person and sign the passport application, form DS-11, before a consular officer.
  2. Complete and sign the:
    Statement Regarding Lost Or Stolen Passport (DS-64) (PDF - 115K).
  3. Bring (if you can) some form of identification (e.g. driver’s license, residence card) as well as documentation that establishes your citizenship (e.g. birth or naturalization certificate, an expired passport).
  4. Bring two identical photos (5cm x 5cm on a white background, there is a photo booth on site).
  5. Pay the appropriate fee.

Replacement passports can be issued within one week to ten days. In cases where the applicant must travel urgently, the Consulate can issue an 'emergency passport' which is valid for three months. This service will not require an appointment, but you must present evidence of your need to travel (train or plane tickets, etc). This passport is valid for all travel, and can be exchanged later for a full validity passport at an embassy or consulate overseas or at a Passport Agency in the United States, free of charge.

Report a lost/stolen passport to the police

A U.S. passport is a valuable document, not just for you the rightful bearer. Therefore always report the loss or theft of a U.S. passport to the local police authorities to avoid Identity theft! How can someone steal your identity? Identity theft occurs when someone uses your personal information such as your name, Social Security number, credit card number or other identifying information, without your permission to commit fraud or other crimes.
