Upon arrival to Dunham US Army Health Clinic, Carlisle Barracks, personnel assigned or attached to DUSAHC must first sign in on the Unit Personnel Register DA Form 647 or DA Form 647-1. Soldiers must sign in with PTMS&E at building 450 Gibner Road, DUSAHC or after normal duty hours report to the Provost Marshall, building 400 Gibner Road, sign in with the Desk Sergeant. Soldiers must retain their DA Form 31 for turn in during in-processing.

Soldiers who request Permissive TDY (PTDY) for house hunting will in-process the DUSAHC PAC and the unit section prior to signing out on PTDY. This ensures that all concerned in the soldier's chain of responsibility are informed.

As we continue to strive to provide the best possible healthcare service to our beneficiaries, Dunham US Army Health Clinic allows you to schedule an appointment for preferentially scheduling patients for appointments the same day they call. This has decreased wasted time waiting to be seen, as well as allows you to usually see your own Primary Care Manager.

It is necessary for service members to call for a same day access appointment. Call (717) 245-3400 or by dialing 106 from any Carlisle Barracks office phone beginning at 0715.

AWC students and faculty may call the day before, after 1500, to make an appointment for the next day to accommodate classroom commitments. Your wait time will be less if you schedule an appointment the day before.
