Passports for Minors
Requirements
Envelope sample
- Completed DS-11 application from the Passport Application Wizard. Do not sign the form. Parents will sign the form in the presence of the Consul as an oath must be administered.
- Certificate of Birth: Certified U.S. or foreign birth certificate with parents' names, or Report of Birth Abroad (Form FS-240) with parents' names, or Adoption Decree with adopting parents' names, or court order establishing custody or guardianship (original or certified copy).
- Applicant’s current passport.
- One photo. The background of the photograph must be white and measure 5cm X 5cm. There is a photo booth at the Embassy that will generate two photos for 5 euros.
- Appropriate fee.

A pre-paid Correio Registado envelope (sample below), size A5 for 100 gr, if you wish to have your new passport mailed to you.
Minors Under Age 16

Parental consent from both parents is required for a passport application of a minor under age 16. Parents may appear in person, with the applicant, to sign the application before the Consul. If one parent is not present in Portugal, the absent parent should complete the Statement of Consent or Special Circumstances: Issuance of a passport to a minor under age 16 (DS 3053) or provide written, notarized permission from the absent parent and a copy of the absent parent’s ID.
