PASSPORT APPLICATION
Provide the following:
1. Passport Application DS-11
a. Click on Applications and Forms under Passports for U.S. Citizens.
b. DS-11 for initial application. This site will not save your inputs.
Submit two separate pages, (not copied/printed side to side) and do not staple.
The Bar Code must be on the application (Top left and bottom right)
2. DD form 1056, No Fee Passport
a. No DSN phone numbers.
b. Must be typed and the original signed in BLUE ink.
Blocks 11, 14, and 15 cannot state “As Directed, World Wide, Various, or To Be Determined.” Must be specific. The must also match the DS-11 exactly.
3. Proof of Citizenship
a. Original Birth Certificate (NO CERTIFIED COPIES)
California Birth Certificates - Long Form
Texas Birth Certificates – Certified
Close to the bottom in the middle, should say “Abstract of Birth
OR
b. Previous Tourist Passport (cannot be expired more than 15 years)
4. Two passport photographs (Do Not Cut)
a. For MARFORPAC contact 477-8588 Combat Camera walk in basis 0800-1100 Monday – Friday located at Basement Level.
b. For USPACOM contact 477-9356 - Visitor Command Center, walk in basis 1300-1600 located in Bldg 700
c. TMO Kaneohe Bay Office contact 257-3566 to make an appointment. Located in Bldg 209, Room 105.
5. Copy of Military/Civilian/DOD Identification Card (front and back)
6. Copy of TAD Orders or PCS Orders to new duty station.
a. If passport is required at new duty station, it must be in the orders.
b. If TAD orders are unavailable provide an Exception Letter in Lieu of Order (See Example)
c. If Passports are required for dependents, both parents must sign for children under 16.
7. Expedite Letter, if passport is required in less than 30 days.(See example)
VISA - Provide the following:
1. DD Form 1056, No Fee Passport/Visa Request
a. No DSN phone numbers.
b. Must be typed and the original signed in BLUE ink.
c. Block 17 (Type in VISA IS REQUIRED, VISA IS ATTACHED and you must have a purpose filled in.)
d. Bring in one original and four copies.
2. Visa Application for Specific country
a. Go to https://www.fcg.pentagon.mil/ to verify entry requirements.
b. Go to http://secureapp2hqda.pentagon.mil/passportmatters. Click on Passport & Visa, pick Country Requirements, find the country and the
application will be under the country information.
c. Most countries require two copies.
3. Photograph
a. Most countries require two.
b. Don’t cut them as each country may require different sizes.
4. Valid Official Passport with signature.
IMPORTANT
1. Turn around for an official passport is approximately 4-6 Weeks. During heavy seasons between February – August it takes approximately 8-10 Weeks.
2. Expedite letters are for departures within 30 working days. Turnaround time for Official Passport with an expedite letter is approximately 15-25 days.
3. A Visa cannot be expedited. The turnaround time for Visa is determined by the embassies.
