Important Notes
1.) It is the sponsor’s responsibility to have changes made in DEERS within 30 days of the change. Two of the most frequent changes needing to be made are address and marital status changes. 2.) All documents must be originals or certified copies.

3.) CAC and ID card renewals are done within 30 days of expiration.

Appointment Guideline
1.) One appointment time per family. 2.) If you are more than five minutes late for your appointment you will need to reschedule or you can wait and be seen as a walk-in customer.

Walk-in Customers
All walk-in customers are seen on a first come, first serve basis. To avoid a possible wait, you should schedule an appointment.

Identification
All customers who are age 21 and older must have two valid forms of ID to receive a new ID card. One must be a state/government issued photo ID. Please visit http://www.uscis.gov/files/form/i-9.pdf for a complete list of acceptable forms.

Dependent ID Cards
The sponsor must be present to sign the DD Form 1172-2. If not available, the dependent must have a DD Fm 1172-2 signed by their sponsor or a Power of Attorney. DD Fm 1172-2 must be signed & witnessed by a DEERS Verifying Official or be notarized.

Dependents age 20 & under need to have at least one valid ID, ex: Dependent ID Card, State, Federal, or Local Government Photo ID, Passport, School ID with photo, Social Security Card, or Birth Certificate.

Adding a Spouse to DEERS
The sponsor needs to bring the following documents for their spouse: Marriage Certificate, Birth Certificate, Social Security Card, & an official Photo ID. A Power of Attorney can be used by the spouse to enroll themselves.

Adding Newborns to DEERS
The sponsor must have the following documents for the child: Proof of Birth Letter or Birth Certificate. Unmarried sponsors must provide Acknowledgement of Paternity with the Birth Certificate (cont)

Adding Newborns to DEERS (cont)
(cont) A Court Order establishing paternity may also be used to enroll a newborn when the sponsor is not married to the mother.
