Physical Exams

Periodic Health Assessment

*Disclaimer: The Periodic Health Assessment (PHA) has replaced the 5 year physical and the Over 40 physical. It is required annually by all Service Members regardless of age.

The Periodic Health Assessment (PHA) consists of three parts. The first part is done online through AKO and is the online questionnaire via AKO. The second part is a walk-in basis by attending the briefing @ 0800 in Kirks Ortiz Training Center (OTC).  At that time will verify the following:

Vision – Exam within the last year

Hearing – TDA units only need to have an exam on file. TO/E units will need a yearly exam

Labs :

HIV within the last 2 years

Lipids- Male soldiers 35 and over and female soldiers 40 and over may need to fast for their Lipids panel. If you are unsure if you need to fast, please contact the Warrior Readiness clinic (410) 278-1960/1950

  

Physicals

Airborne, Ranger, Special Forces, SERE, Commission, Warrant Officer

To begin the process of getting a physical for any of the above listed physicals, service members will need to attend a briefing that begins @ 0800 daily in the OTC, KUSAHC, Building 2501, APG.

The paperwork needed for the above physicals is listed below and located on the right hand side of the web page under Warrior Readiness forms. Once it is filled out please email it back to KirkWarriorReadiness@amedd.army.mil before your walk-in/appointment date:

Separation Exams

To begin the process of getting a separation physical soldiers need to will need to attend a briefing that begins @ 0800 daily in the OTC, KUSAHC, Building 2501, APG.

For Service members that are being involuntarily chaptered are required to have an NCO excort throughout the process for the final appointment.  They must also provide a memo from their commander stating what chapter they are being process under.  The memo can be emailed to KirkWarriorReadiness@amedd.army.mil prior to arriving at KUSAHC, or it can be brought in during the walk-in portion. 

Retirement Physicals

Service members can begin the Retirement physical 6 months prior to their transition/retirement date.

To begin the process of getting a separation physical soldiers need to will need to attend a briefing that begins @ 0800 daily in the OTC, KUSAHC, Building 2501, APG.

Complete these forms and email them to KirkWarriorReadiness@amedd.army.mil prior to your walk-in/appointment date. These forms are located in the upper right hand side of this page.

Reserve and National Guard soldiers will need to bring with them their ID card and a set of orders activating them to receive care, or a memo from their commander authorizing care on that specific day. For more information please contact Warrior Readiness staff – (410) 278-1960/1950

Allow for a minimum of 5-10 business days for all physicals to be signed by a medical Doctor. Once Warrior Readiness has received the complete and signed physicals the staff will contact the Service Member to come pick up the completed physical
