About the Program
Geriatrics/Extended Care provides a spectrum of services for eligible veterans enrolled in the system. Some of the services are provided for inpatients admitted to the nursing home care unit (Building 2). The majority of care/treatment/services are provided in outpatient programs to support the wishes of the patient to live at home or in their communities as long as possible.

- Specialized rehabilitation following amputation, stroke, injury, surgery, or chronic mental health or medical illnesses
- Home-Based Primary Care

- Contract Adult Day Health Care
Outpatient Care:
- Home-Based Primary Care

- Community Nursing Home Care
- Skilled and Unskilled Home Care
- Home Respite Care
- Home Hospice Care
- Community Residential Care
- Homemaker/Home Health Aide Service
