School Health Assessment/Sports Physical

Medication at school..Does your student require daily medication? Do they use an asthma inhaler, or another form of medication which is used periodically? Medication cannot be carried by students, or kept in the health office, without physician authorization being on file with the school nurse. Please contact the health office for more information at 065.44.5757.

Immunizations..Was the Immunization Clinic "out of stock" when you tried to get your student their DoDEA required immunizations? Don't forget to follow-up and take your student back to the clinic when stock is replenished. For more information, please contact the Immunization Clinic at 065.44.5848

Sports Physicals..If your student plans to participate in athletics, parents must submit a current sports physical to the sponsor of whatever sport your student is participating in. After being seen in the clinic, clinic personnel will provide you with a copy of the sports physical, be sure to make an extra copy for submission with other informational/parental authorization documents found in the sports packet. The Wellness Plan includes policy information on food and beverages sold on school campus, as well as physical activity and education requirements.
