Please prepare the following documents when registering your child. Documents are categorized: Mandatory Forms, Bus Transportation, School Mandatory Forms, and Optional forms. Mandatory documents/forms are required by all schools.
MANDATORY DOCUMENTS
____ Permanent Change of Station (PCS) orders/Certification Letter (required for both new and returning students)
· Active Duty Member: Military Orders (and Page 2 for Navy)
· Army or Air National Guard: Military Orders and Certification Letter, if orders read, "Unless sooner released..."
· DoD Federal Employee to include DoDEA (CONUS hire): PCS Travel Orders, if orders expire provide Renewal Agreement Travel (RAT) orders during re-registration week
____ DoD Federal Employee to include DoDEA (Local hire): Certification Letter
____ Birth Certificate (copy required for PreKn-1st), Passport (required for new students)
____ Immunization/Shot Record or Card OR DoDEA form 2942.O-M-F1 "DoDEA Immunization Requirements" (required for new students) 2 page document download link
____ Court document if sponsor is legal guardian of child with required DSO clearance (required for new students)
