The Health Care for Homeless Veterans (HCHV) program’s goal is to end homelessness among veterans through outreach efforts and community partnerships. Veterans are engaged in treatment and rehabilitation programs that enable them to achieve a better quality of life.

HCHV program is designed to address the needs of homeless veterans. HCHV staff work with veterans who are mentally ill or have substance abuse problems, to help them overcome the barriers that may prevent them from receiving or benefiting from health services. Reaching out to homeless veterans in shelters, soup kitchens, and on the streets, VA outreach workers help establish eligibility for VA medical services, develop appropriate treatment plans, and screen for community placement.

VA staff connects with shelters, community organizations, and local governments to provide veterans with access to these combined resources. Through these connections, the HCHV program strives to serve all homeless veterans in our area and improve their quality of life.
