Welcome! Thank you for choosing the Department of Veterans Affairs Robert J. Dole VA Medical Center (VAMC) as your clinical training/education site. For over 75 years, the Dole VA Medical Center has been honored to serve Kansas area Veterans. The Dole VA provides a full range of primary, specialty, acute and extended care services to Veterans in 59 counties of Kansas. To aid in extending our care beyond the Wichita area, we have six Community Based Outpatient Clinics (CBOC's) located strategically throughout Kansas to provide care to those Veterans living in rural areas. The driving force behind the high quality of care delivered at the Dole VA is our mission statement:

Below you will find all required documents along with instructions on how to properly complete them. You will also find directions on how to create an account in the Talent Management System (TMS) so that you can complete the course entitled “Mandatory Training for Trainees”. Once you have completed all documents there are 2 ways in which to submit these to your point of contact.

  1. If you are part of a group of students then print all documents and submit them to your instructor or Clinical Site Coordinator so that they can send in all documents together to your VA point of contact. This can be done electronically or via mail.

  1. If you are a student that is not part of a group of students then you may submit your paperwork directly to your VA point of contact. The preferred method is to send these documents via email, if this is not possible then you may mail in the documents.

 

First you or your clinical instructor/Clinical Site Coordinator will notify your VA point of contact of your anticipated start date. You must contact the Human Resources Department to complete fingerprinting. Please contact the HR Department at (316) 685-2221 x 53272 to set up an appointment. This needs to be done at least one month prior to your anticipated start date. Once you have scheduled an appointment for fingerprinting you will need to complete the form VA 0711. This document must be taken with you to your appointment with HR along with 2 forms of identification which are outlined on the VA Form 0711 on page number 5.

Open the above VA Form 0711 by clicking on it and fill out the sections indicated below. This form needs to be submitted to your VA point of contact and also taken with you to the HR department for your fingerprinting.

Without Compensation Form (WOC)

 

Instructions

Open the above Without Compensation Form (WOC) by clicking on it. Please fill out all blanks electronically if possible. If you are unable to complete the forms electronically, then print the documents and fill in the rest of the spaces using a pen. Please ensure that all information is legible.

The Without Compensation (WOC) form must have the following information: Required information is marked in red on the form.

a) Your full name at the top of page 1 where indicated.

b) Clinical start date and end date.

c) Social Security Number

d) Date of Birth

e) Signature

f) Date completed

g) Veteran Status

h) School Affiliation (name of your school/university)

i) Type of program

j) Program Title (degree title)

k) WOC Position Title (i.e., Student Nurse, Student Nurse Practitioner, Student Physician Assistant, etc.)

l) Approximate total hours scheduled to be at VAMC.

Below you will find a map of the Robert J Dole VAMC Campus. On this map you will see that the Education Department and Human Resources Department have been identified in red. You will also find the main entrance of the hospital indicated by a red square. This is a standard place for you and your VA POC to meet on your first day here at the facility.

 

Also on the map please note the parking areas that are reserved for employees. These are the areas in which you will be expected to park in unless otherwise instructed by your VA POC.
