To the medical center:

From Interstate 17

Take the Indian School Road east exit. Turn left [north] at the 3rd Street light.

From State Route 51, Piestewa Peak Parkway

Take the Indian School Road west exit. Turn right [north] at the 3rd Street light.

From Interstate 10

Take the 7th Street exit, heading north. Turn left at Indian School Road. Turn right [north] at the 3rd Street light.

From 3rd Street, take the first right into the Ambulatory Care Center parking lot.

Patient Transportation Services

Travel Eligibility Criteria

Patient Transportation Services provides limited transportation service to eligible veterans in need of medical care.  Veterans living beyond a 27-mile radius of the Carl T. Hayden VAMC may be reimbursed if they meet the eligibility requirements as outlined below. The veteran is responsible for the first $6 with a monthly maximum of $18. Travel by specialized mode such as ambulance, wheelchair van, etc., will only be authorized when a VA physician recommends such transportation is medically required.

  • A veteran traveling for treatment due to a service-connected disability.
  • A veteran with a service-connected disability rated at 30% or more, for treatment of any condition.
  • A veteran receiving VA pension benefits.
  • A veteran whose annual income does not exceed the maximum annual rate of pension, which would be payable if the veteran was eligible for pension.

Veterans are urged to arrange transportation five working days in advance.  Veterans may schedule transportation with the transportation dispatcher between the hours of 8:00 a.m. and Noon.
