IN-PROCESSING

OUT-PROCESSING - A copy of your orders is required.  Medical records can no longer be hand carried to your next duty station; they will be forwarded according to AR-40-66.

REQUESTS FOR MEDICAL RECORDS from another Military Treatment Facility or civilian providers (please provide the provider's name, address, phone number, and fax number if possible).

Release of Medical Records Form

REQUESTS FOR COPIES OF MEDICAL RECORDS: We need a signed request for all copies of medical records.  Anyone over the age of 18yrs old MUST sign the request; this includes spouses and dependents unless there is an authorization form on file giving permission for someone else to sign for the patient.  Records are copied in order by date the request was submitted; we currently have at least a 4 week waiting list.  We are only authorized to make 1 copy but you can request a printout of any new appointments.  Request for copies of behavioral health records must be requested from the Behavioral Health Clinic located on the 3rd floor. 

LABS: Patients may request a copy of their labs from either Medical Records or Patient Administration.  If you need labs faxed to another provider, you will be required to sign a release of information form authorizing the release of information.  Again, anyone over the age of 18 yrs old MUST sign the request; this included spouses and dependents unless there is an authorization form on file giving permission for someone else to sign for the patient.

PRE-OPS: Following your appointment for you pre-op history and physical, please bring all documentation to the Patient Administration office to be faxed to the provider and/or hospital.

MEDICAL AND DENTAL PREPARATION FOR OVERSEAS REASSIGNMENT (POR): All personnel PCSing overseas are required to attend a briefing which is held every Tuesday at the clinic in the OTC room.  If your family is going with you, please contact the Chief of Patient Administration at (410) 278-1720 as soon as possible.

REMINDER: If you have a family member in the Exceptional Family Member Program, the enrollment must be updated every 3 years.
