Forms I-864, I-864EZ, I-864A, and I-864W

You should be viewing this page if:

  • You are sponsoring an intending immigrant and…
  • You are required to submit an Affidavit of Support.

Follow the guidance in the table below to determine the appropriate Affidavit of Support form for your situation.

IF…

THEN you need to…

The applicant you are sponsoring has earned or can be credited with at least 40 eligible quarters under the Social Security Act (SSA)

  1. Complete an I-864W on your computer using CAPITAL letters
  2. Review the I-864W Checklist
  3. Print the completed form
  4. Attach the bar code sheet, and
  5. Mail them to the NVC unless you have joined the E-Mail Electronic Processing program, then…
  6. Scan the I-864W to Portable Document Format (PDF) and send it as an e-mail attachment to NVCElectronic@state.gov. Sending physical documents to the NVC will delay the processing of your case. When sending completed forms to the NVC do not include instruction pages or any other sheets which do not carry information you entered while completing the form.

You are sponsoring a child who will become a U.S. citizen immediately upon entry under the Child Citizenship Act of 2000 (CCA)

 

  1. Complete an I-864W on your computer using CAPITAL letters
  2. Review the I-864W Checklist
  3. Print the completed form
  4. Attach the bar code sheet, and
  5. Mail them to the NVC unless you have joined the E-Mail Electronic Processing program, then…
  6.  Scan the I-864W to Portable Document Format (PDF) and send it as an e-mail attachment to NVCElectronic@state.gov. Sending physical documents to the NVC will delay the processing of your case. When sending completed forms to the NVC do not include instruction pages or any other sheets which do not carry information you entered while completing the form.

You are a self-petitioning widow(er)

 

  1. Complete an I-864W on your computer using CAPITAL letters
  2. Review the I-864W Checklist
  3. Print the completed form
  4. Attach the bar code sheet, and
  5. Mail them to the NVC unless you have joined the E-Mail Electronic Processing program, then…
  6.  Scan the I-864W to Portable Document Format (PDF) and send it as an e-mail attachment to NVCElectronic@state.gov. Sending physical documents to the NVC will delay the processing of your case. You are a self-petitioning battered spouse or child

 

  1. Complete an I-864W on your computer using CAPITAL letters
  2. Review the  I-864W Checklist
  3. Print the completed form
  4. Attach the bar code sheet, and
  5. Mail them to the NVC unless you have joined the E-Mail Electronic Processing program, then…
  6. Scan the I-864W to Portable Document Format (PDF) and send it as an e-mail attachment to NVCElectronic@state.gov. Sending physical documents to the NVC will delay the processing of your case. When sending completed forms to the NVC do not include instruction pages or any other sheets which do not carry information you entered while completing the form.

All three of the following are true:

  • You filed a Form I-130 petition, for your relative
  • There is only one applicant on the Form I-130 petition, and
  • All your qualifying income comes from a salary or pension and shows on one or more W-2 Forms that you receive 
  1. Complete an I-864EZ. on your computer using CAPITAL letters
  2. Review the I-864EZ Checklist
  3. Print the completed form
  4. Attach the bar code sheet, and
  5. Mail them to the NVC unless you have joined the E-Mail Electronic Processing program, then…
  6. Scan the I-864EZ to Portable Document Format (PDF) and send it as an e-mail attachment to NVCElectronic@state.gov. Sending physical documents to the NVC will delay the processing of your case. When sending completed forms to the NVC do not include instruction pages or any other sheets which do not carry information you entered while completing the form.

If none of the above apply

  1. Complete an I-864 (and possibly an I-864A) on your computer using CAPITAL letters.

Note: you need form I-864P to complete the I-864

  1. Review the Form I-864 Checklist (and possibly the Joint Sponsor’s Checklist and the Household Member’s Checklist)
  2. Print the completed form(s), and
  3. Attach the bar code sheet and mail them to the NVC unless you have joined the E-Mail Electronic Processing program, then… 
  4. Scan the I-864 to Portable Document Format (PDF) and send it as an e-mail attachment to NVCElectronic@state.gov. Sending physical documents to the NVC will delay the processing of your case. When sending completed forms to the NVC do not include instruction pages or any other sheets which do not carry information you entered while completing the form.
